DIRECTIONS: Please read and initial each statement below to signify you understand and agree to the terms.

1.) There is no way to guarantee color results prior to tanning. *

2.) Some people may be allergic to the tanning solution. Sunny Girl Spray Tan ATX is not responsible for damages because of reactions to the product. Product ingredients may be requested at any time. *

3.) The time a tan will last will depend on the individual’s skin type and how well the tan is cared for after tanning. Individual results will vary. *

4.) I have read and understand the “do’s and don’ts” provided by Sunny Girl Spray Tan ATX. *

5.) I understand that any part of my body that comes into contact with tanning solutions will be tanned. Do NOT touch yourself until you are dry. Sunny Girl Spray Tan ATX is not responsible for tanning spots in places you were not tanned (palms, underarms etc.) *

6.) Full-figured persons will have untanned areas in their natural body creases. *

7.) Be sure you are dry before allowing your legs to touch or your arms to fall to your sides. Color variations are difficult to avoid in this situation. *

8.) Sunny Girl Spray Tan ATX is not responsible for lost or stolen items. *

9.) I hold Sunny Girl Spray Tan ATX and its owner, innocent of any charges or financial claims. *

10.) If photos/media is taken: I hereby grant permission to Sunny Girl Spray Tan ATX to take and use: photographs and/or digital images of me for use in news releases and/or promotional materials. These materials might include printed or electronic publications, web sites, or other electronic communications. I further agree that my name and identity may be revealed in descriptive text or commentary in connection with the image(s). I authorize the use of these images without compensation to me. *
